DEPARTMENT OF TRANSPORTATION (DOT) EXAMINATIONS
Dr. Jeffrey Dysart is registered as a Certified Medical Examiner by the National Registry of Certified Medical Examiners to perform DOT examinations.
Effective May 24, 2014, we charge $100.00 for this exam. We offer examinations by appointment Monday - Friday during all office hours. We strive to offer you appointments within one business day.
Please note, this is an administrative physical and is generally not covered by medical insurance. We are unable to bill your carrier for this service. You must pay in full prior to examination.
Prior to your appointment, go to the Federal Registry of Certified Medical Examiner's web site at https://www.dmv.ca.gov/forms/dl/dl51.htm to download and complete your portion of the Medical Examination Report (DL51). Fill out the driver portions of the form, print, and bring to your appointment along with a current driver's license for identification. Dr. Dysart will perform your exam, fill out the examiner portion of the form, and issue your updated certificate as appropriate. Drivers are responsible for filing the DL51 with the Department of Motor Vehicles.
