USCIS MEDICAL EXAMS (Immigration Exams)

Effective October 01, 2021, USCIS will REQUIRE proof of completed COVID19 vaccination for all eligible applicants at the time of your medical examination. As of July 25, 2022, this includes all applicants ages 6 months and up. To find a vaccination clinic and schedule an appointment, please visit My Turn (https://myturn.ca.gov) Please note, if receiving Pfizer or Moderna, it will take 3-4 weeks to complete your vaccination series.


Dr. Jeffrey H. Dysart is a designated Civil Surgeon for the U.S. Citizenship and Immigration Services (USCIS) through the Department of Homeland Security.


$275.00 - must be paid in full at time of visit; includes all visits and required blood tests (RPR, QuintiFERON (TB)). If further tests or vaccinations are indicated, they will be offered at additional charge.

Children under age 15 will receive a discount for any test that is not required.

There will be a $50.00 charge for completing additional sets of paperwork past the originals provided.


USCIS exams consists of a minimum of 2 visits, generally about a week apart. The first visit consists of a review of your vaccination documentation, a detailed history and a physical exam. It is recommended that you have your vaccination history ready for the first appointment. The second appointment is to review your lab results and complete the application.

To prepare for your appointment, visit the USCIS website at http://www.uscis.gov. A download of the I-693 application is available on their site.


For your appointment please bring:

  • Your completed I-693

  • DO NOT sign your I-693; your signature must be completed in front of the physician! The physician will complete the Civil Surgeon-required portions of your application. The original documents will be placed in a sealed envelope for USCIS and a copy of the documents will be given to you.

  • A valid passport from your country of origin
  • Proof of required vaccines
