USCIS MEDICAL EXAMS (Immigration Exams)
*** As of 10/15/2016, we have had to increase our prices due to additional lab tests required by the USCIS.
$175.00 - must be paid in full at time of visit; includes required blood tests and PPD test. If indicated, vaccinations, titers for vaccination status and chest x-rays are not included in this price.
Children under age 15 will receive a $25.00 discount.
There will be a $50.00 charge for completing additional sets of paperwork past the originals provided.
Dr. Jeffrey H. Dysart is a designated Civil Surgeon for the U.S. Citizenship and Immigration Services (USCIS) through the Department of Homeland Security. We offer exam times daily, morning and afternoon, with the exception of Thursdays; USCIS exams require a skin TB test which must be read by our physician two days later. Exams are done by appointment only.
To prepare for your appointment, visit the USCIS website at http://www.uscis.gov.
There you will electronically fill out the I-693 form required for your appointment. Bring your completed I-693 and your legal identification to your appointment. DO NOT sign your I-693; your signature must be completed in front of the physician. The physician will complete the Civil Surgeon-required portions of your application. The original documents will be placed in a sealed envelope for USCIS and a copy of the documents will be given to you.

Please note: In order to complete your examination, you will need to visit our office twice. At the first appointment, we will conduct your examination and any required laboratory testing. The second appointment will be scheduled for two days later in order to read your tuberculosis test and complete your documents. The initial cost covers both appointments.
